CASE MANAGER AT MONDIA SUNNYSIDE – GQEBERHA (PORT ELIZABETH)

We’re seeking a dedicated Case Manager to join our dynamic team at Mondia Sunnyside, based in Gqeberha (Port Elizabeth).

REQUIREMENTS, EXPERIENCE AND QUALIFICATIONS/TRAINING

  • Diploma OR Degree in Clinical Field advantageous
  • 2 years or more relevant case management experience in a Mental Health environment advantageous
  • Knowledge and understanding of all funders
  • Computer literate with strong administration and organizational skills
  • An understanding of current coding structures in the SA Health Care Environment

the Eastern Cape reporting to the Unit Manager.
REQUIREMENTS, EXPERIENCE AND QUALIFICATIONS/TRAINING
 Care Worker certificate.
 2 years clinical experience.
 Must have received or be willing to receive the COVID-19 vaccination by date of hire to be
considered. Proof of vaccination required.
MAJOR DUTIES / RESPONSIBILITIES
 To contribute to the best possible outcome for patients while taking care of their basic needs
by using a person-centred approach.
 Identify and report risk and ensure the highest possible quality standards according to policies,
processes and tasks and LEAN principles.
